Name changes due to incorrect spelling or legal situations can be requested by completing the printable Name Change Request Form and faxing to (302)831-3005 or mailing to the address on the form.
Requests must be accompanied by appropriate legal documentation of the new name. Please provide a photocopy of your driver's license or other documentation of name such as birth certificate, marriage license, divorce decree or court order.
