The Graduate College confirms completion of degree requirements for graduate students and individual colleges confirm completion of degree requirements for undergraduate students.     Forms that require the Registrar’s signature/seal: Do not drop off or use email to submit forms. They can be securely uploaded (and connected to your student record) with any instructions to our secure server using this link.

    Enrollment/Degree Verifications: Enrollment verifications are available via student self-service in UDSIS. Please visit the Enrollment Verification page for more detailed information.

    Transfer Credit Evaluation (TCE) Forms: Please visit our How to Transfer Undergraduate Courses to UD page for information on how to submit your TCE form. For additional information visit our Transfer and Test Credit page.

    Ordering Official Transcripts: Current and former students may order transcripts via their UDSIS Student Center or by clicking on this link.  Please see the Official Transcript page for more detailed information.

    Apostille/Notary Services: Please visit the Notary/Apostille page for instructions on how to make a request, how to submit materials, and, if needed, to schedule an appointment.

    Federal Investigators: in need of enrollment/degree information for current and/or former students, please click here to schedule an appointment.

    Registration Holds: If you have a registration hold on your student record, please visit this site for instructions how to determine what the hold is for and how to remove it.

    Diplomas: Concerns regarding damaged or missing diplomas and questions regarding Latin honors should be emailed to Please include your full name and UDID in your inquiry.

    Registration Concerns: If you do not meet the enrollment restrictions for a course (i.e., reserved seats or requisites), please contact the academic department directly.
