International Student & Scholar Services Forms

This section provides a list of campus and community resources that can help international students address common issues faced during their stay at UD and in the United States.

New Student Forms

To update your local address in the U.S., make sure your mailing address in UDSIS is correct. This address must remain a U.S. address. Updating any other address in UDSIS will not result in an update to CGPS and the government. It is your responsibility to update your mailing address within 10 days of the change.
To complete this form, students must: 1. Accept your offer of admission on your application status page. 2. Complete the "Request for I-20/DS-2019" form online. Undergraduate students can access this form in their Blue Hen Portal and Graduate students can access this form from their Status Page. 3. Upload your proof of funding or financial documents, Affidavit of Support (if needed), passport, and any passports for dependents.
Submit this form if your funding source is from someone other than yourself, your government, or the University of Delaware (for example, family funds). (Note: This form is fillable.)
Complete, print, and sign this form and provide it to the international student advisor at your current school processing your SEVIS transfer to UD. When complete, please upload this form in your I-20/DS-2019 Request. (Note: This form is fillable.)
Estimated costs of attendance for one academic year at UD.

Student Employment - F-1 Optional Practical Training (OPT) Forms

For more information about OPT.
This application is for graduate students ONLY.
This application is for undergraduate students ONLY.
Use this form to notify our office if you have new employment or to report any employment updates. (This form can be used to update both employment and home address.)
Use this form to update your address with our records while you are on OPT.
This application is for those seeking to apply for the OPT STEM Extension.

Student Employment - F-1 Curricular Practical Training (CPT) Forms

As a part of this online application, your academic advisor will need to fill out an electronic recommendation. As a graduate student, you will also need electronic approval from the Graduate College.

Student Employment - J-1 Academic Training Forms

For more information about Academic Training.
In order to submit this application, you will need an offer letter as well as a recommendation letter from your academic advisor. Academic Training must be authorized before you are eligible to start employment.

Student Employment - J-1 On-Campus Employment Forms

For more information about On-Campus Employment.
This application is for J-1 Students ONLY. This form is to report your on-campus employment to CGPS. On-campus employment for J-1 student visa holders can be authorized for up to 12 months. You must renew your authorization for the on-campus employment every 12 months (or if you end/change employment) by completing this form.

Student Employment - Social Security Forms

Use this application to apply for an original or replacement Social Security card and to change or correct information on your Social Security number record.
Use this link to complete the online application and download the On-Campus Employment Eligibility form. Approval from CGPS is needed on this form before going to SSA.

Student I-20/DS-2019 Updates/Extensions

Use this application if you have any changes to your program, such as adding or changing a major.
Use this application if you need to extend your I-20/DS-2019.

Additional Student Forms

To update your local address in the U.S., update your mailing address in UDSIS. This address must remain a U.S. address. Updating any other address in UDSIS will not result in an update to CGPS and the government. It is your responsibility to update your information within 10 days of the change.
Submit this application with your I-20/DS-2019 each time you plan to travel outside the U.S. If you have left the U.S. without a travel signature, you can request a reprint of your document with a travel signature using this form (you will need to register for express shipping to have it shipped to you).
Submit this application if you will be enrolled less than full time (only allowed one time per academic level in addition to the last semester of study if needed). Those requesting Medical Reduced Courseload should also complete their request via this application.
Use this application to submit a request to transfer your SEVIS record to another school.
Submit this application to request a new I-20/DS-2019 for dependent(s).
Use this form if you are a student at any institution interested in taking part-time classes at UD to count toward your minimum requirement. Only use this form if you are taking part-time classes at UD and part-time classes at another institution. Do not use this form for reduced course load or if you have been dismissed from UD and do not have a valid I-20. Only students who have a valid I-20 in a matriculated program are eligible to use this form. This form must be completed before you engage in dual enrollment. Engaging in dual enrollment without authorization is considered a violation of your status.
This form must be completed by a UD staff or faculty member who is providing translation of documents into English.
(Note: This form is fillable.) This form must be completed for any request for Honorarium or Payment for Services of a Non-U.S. citizen before contacting Procurement to complete the W-8. Please submit this form with the appropriate documents shown on page 2. It is the responsibility of the engaging department to complete this form first. Procurement Services will assume that this has been completed if they receive a request for supplier registration and payment. This form is not required to reimburse for travel and living expenses. For more information, please contact Procurement Services.

LETTER REQUEST FORMS

 

Invitation letters

These letters are not required by the embassy to issue B-1/B-2 visas. The Student/Scholar should provide a copy of their I-20/DS-2019 to the relative applying for the tourist visa. You can also send an Enrollment Verification Certificate along with the copy of the I-20/DS-2019 (for students only).

 

Status letters

You can print an Enrollment Verification Certificate directly from your UDSIS student page. This letter along with a copy of your I-20 or DS-2019 should suffice in proving your status.

 

Other Letter Requests

Please email oiss@udel.edu regarding requests for any other letters.

Enrollment Verification Certificates can be printed online from your UDSIS page.

For funding letters, please contact your Department.

The following forms are used for ELI students who need to submit the appropriate requests below.
Please note: You must use your UDEL username and password. You will be required to type in your 2 Factor Authenticator code. You must click on the green "Submit Application" button in Terra Dotta to complete your request.

ELI STUDENTS

 

ELI Change of Level: Pre-AT to AT

If you have met your Pre-AT requirements and it has been approved by the ELI and you need a new Pathway I-20, use this application to submit your request.

Complete this application if you need to request a new I-20/DS-2019 because your document was lost, damaged, or stolen.

ELI Medical Leave of Absence

Submit this application if you are unable to attend classes full time due to a medical condition. A doctor’s letter is also required in the application.

ELI Student SEVIS Transfer-In Form

Complete, print, and sign this form and provide it to the international student advisor at your current school processing your SEVIS transfer to UD. When complete, please upload this form in your I-20/DS-2019 Request.

ELI Transfer Out Form

Use this application to submit a request to transfer your SEVIS record to another school.

Please read through the information on the ISSS/Departments page and then contact CGPS to be connected with the appropriate ISSS advisor. After an initial consultation you will be provided with the application form to collect information that will allow us to help you build an outline for a sustainable international program.
