Instructions: The Performance Evaluation Form is the instrument used to evaluate a student’s performance and assign a project grade. A project will be considered complete when the form is filled out, signed, and a discussion of the evaluation has been held with the Fellow and the Project Leader. The completed Evaluation Form should provide a basis for the brief and frank discussion between the Project Leader and the Fellow. The Project Leader is encouraged to provide constructive criticism and helpful comments to the Fellow. The goals of this process are to promote better self-understanding and improved performance of the Fellow.
The grade for the project will be determined: 86-90 points............= A
81-85 points............= A-
78-80 points............= B+
76-77 points............= B
72-75 points............= B-
Points less than 72 are unacceptable
Procedure:
1. Either the Project Leader or Coordinator may request changes/corrections be made in content or format of the project’s final project before copies are distributed.
2. The Performance Evaluation Form will be completed by the Project Leader.

A. Communication: Did the Fellow communicate in writing in a clear, succinct, informative, and interpretive way? Did the Fellow communicate orally in a clear, articulate and timely manner? Did the Fellow communicate “vertically” and “horizontally” as needed? Was the Fellow a good listener?

Instructions: The Performance Evaluation Form is the instrument used to evaluate a student’s performance and assign a project grade. A project will be considered complete when the form is filled out, signed, and a discussion has been held with the Fellow and the Project Leader. The completed Performance Evaluation Form may provide a basis for a brief and frank discussion between Fellow, Project Leader and Coordinator. The goals of the process are to promote better self-understanding and improved future performance.
The evaluator (project leader) is responsible for an honest and fair appraisal of the Fellow's performance on the project. The evaluator is encouraged to provide constructive criticism and helpful comments for the Fellow, who has goals to broaden experiences and improve leadership skills.
The grade for the project will be determined: 68-72 points............= A
65-67 points............= A-
63-64 points............= B+
60-62 points............= B
58-59 points............= B-
Points less than 58 are unacceptable
Procedure: l. Program Coordinator will forward Evaluation Form and approved Project Form to the project leader before July 15, 2002.
2. Project leader will evaluate the Fellow throughout the summer, complete the form, and forward the form to the Coordinator.
3. Either the Project Leader or Coordinator may request that changes/ corrections in content or format be made before copies are distributed.
