Group term life insurance is offered through the flexible benefits program to all full-time and part-time faculty members, except for temporary employees in their first year of service and employees with a visiting classification. There is no cost to the individual for insured amounts up to two times the annual base salary. Individuals who select four times the base salary pay the difference. The program provides various levels of insurance in accordance with an individual's choice. It is canceled upon termination of service, except that within 31 days of termination of employment, the full amount or a portion thereof may be converted to an individual policy upon application to the insurance company and payment of current rates. Conversion forms are available from the Benefits Office.
