Application Procedures
- Applications are available in the office hallway of the Fred
Rust Arena, the Lobby at the Pond and
online in PDF format.
- If you are unable to obtain an application ask your coach to
obtain one for you or call the club office at (302) 831-4686.
- All applications must be accurately completed and signed by your
coach.
- All appropriate fees, in the form of check or money order
payable to UDFSC, must accompany your application (See reverse
for test fees and see below for additional fees)
- Incomplete applications or applications without appropriate fees
will be returned and your test will not be scheduled.

- Applications must be received by 5:00 PM in the UDFSC mail lock
box on or before the date specified on the current published test
schedule. The deadline is two weeks prior to the date of the test
session and will be posted on the bulletin board in the Rust
Arena.
Fees in addition to regular test fees
- Non Members of UDFSC must pay an additional $30.00 non member
fee per test session.
- Late applications may be turned in with a $25.00 late fee.
PLEASE NOTE THAT THIS DOES NOT GUARANTEE YOUR TEST CAN BE SCHEDULED.
- Fees for a specially scheduled test session will be based upon a
pro rata share of the actual test session expenses if regular
fees are inadequate to cover costs.

Test applicants should heed the following advice before
submitting an application
- Test fees are non refundable so be sure that you are
available to take the test at any time on the scheduled
day.
- Testers and coaches should be prepared to test up to one
hour before their scheduled time. If the test session runs
early and you are not present one hour before your
scheduled time, you risk forfeiting your test.
- When submitting an application for a contingent test, be
aware that your test fee is forfeited if you are unable to
take the contingent test.
- Scheduling of tests on applications submitted after the
test schedule is posted is subject to time restraints and
judges availability. If the test cannot be scheduled, it
will be forwarded to the next test session. The fees are
non-refundable.
- Scheduling the tests involves many variables; individual
needs cannot always be accommodated. Think carefully before
submitting an application for testing on a day that you
have other commitments.
- If UDFSC is not your home club, a letter of permission from
your home club must be received prior to the test date or
you will not be allowed to test. Permission letters may be
submitted with the application, faxed to (302) 831-2812, or
e-mailed to UDTestChair@aol.com.
- If you have any inquiries or special circumstances, please
indicate on the reverse side or attach a note to the
application. Once the schedule is posted there will be no
rescheduling to accommodate individual needs.
USFSA Membership
- All applicants must, at the time of the test, be a member in
good standing of the USFSA. Your USFSA number must appear on the
test application on the reverse side of this sheet. Put "applied
for" in the space requesting your USFSA number if you are a new
member and have not yet received your USFSA card.
- Anyone taking a test without a current USFSA membership becomes
a defacto UDFSC member, and all USFSA/UDFSC membership dues must
be paid immediately.
Permission Letters
- If the UDFSC is not your home club, you must attach a letter of
permission from your home club indicating that you are a member
in good standing and have permission to test at the UDFSC.
- If you are taking USFSA tests at other clubs, you must get a
letter of permission from the UDFSC to verify that you are a
member in good standing of the UDFSC or use a copy of the "good
standing" letter provided to all UDFSC members.
Delivering your application
Put your application in the UDFSC Mail Lock Box located
above the mail cubbies in the office hallway in the Fred Rust
Arena by 5:00 PM on the specified date. Hand in your own
application -- DO NOT ENTRUST OTHERS TO HAND IT IN FOR YOU.
