As a patient you have choices, rights and responsibilities. We are concerned with providing you the very best in health care and ask that you participate in your own health care by being an active and informed consumer.



You Have The Right To:

Dignity Privacy Confidentiality
An Explanation of Your Care
(including diagnosis, treatment options and prognosis)
Know Availability of Services
Know All Fees For Service
Know Names and Positions of People Serving You Review Your Medical Records and/or Receive a Copy Request Corrections to Your Medical Record
Have Your Medical Records Released Only with Your Permission
Request to Change Health Care Providers If Available
Accept or Decline Care/Treatment

You Have the Responsibility To:

Be Honest About Your Medical History - to include hospitalizations; surgery; medications, including over-the- counter products & dietary supplements; and any allergies or sensitivities
Report Any Changes in Your Health
Understand the Health Care and Services Offered
Follow Agreed Upon Health Care and Medical Instructions
Accept responsibility for any charges not covered by your SHS
fee (or your insurance)
Respect Other Students and Student Health Personnel
Make/Keep/Cancel Your Appointment
Provide Us with Feedback for Improving Services*

There are limited confidentiality exceptions required by law, such as reporting certain communicable diseases, or situations which threaten your own safety or safety of others, and age of consent
