Dear Admitted Students:

Congratulations on your admission to the University of Delaware! The Student Health staff look forward to the opportunity of providing the primary medical care and health information you need to successfully complete your college education. So that we can provide you the best care possible, you are required to return the Personal and Family Medical History Form and the Immunization Record Form. You should receive these forms in the mail. You can get detailed information about immunization requirements on our page concerning pre matriculation immunization requirements . If you have difficulty receiving the forms by mail, you can also download copies of the forms from the pre matriculation immunization forms page . If you are admitted for the fall term, please return the forms in June. If you are admitted for the spring term, please return the forms in January. If you are admitted late, please return the forms as soon as possible but before classes begin. You will not be permitted to register for the next semester if forms are not completed and returned.

You will see by exploring our pages that we provide many services for you, including twenty-four hour seven day inpatient and outpatient nursing care during the fall, winter, and spring semesters whenever the residence halls are open (weekday hours during Summer and Spring Break), practitioners and nurse practitioners on-site daily and available by telephone to the nursing staff whenever Student Health is open, a Gynecology Clinic which offers women annual exams including Pap smears (thin preps) as well as treatment of many gynecological problems, an Allergy/Immunization Clinic, a Sports Medicine Center which is available to NCAA athletes and to all students by referral from a Student Health practitioner, Nutrition Services, Clinical Lab, X-ray Services (billed to your insurance), and a Dispensary which you may elect to use for common pharmaceuticals and orthopedic supplies ordered by a Student Health practitioner.

Whereas Student Health provides many services to you, you will still need health insurance to cover services beyond those offered on campus. If you do not have health insurance coverage, the University sponsors a sickness and accident plan at a reasonable cost. If you are an international student, you will be required to enroll in this plan or an equivalent U.S. based plan. We look forward to seeing you during orientation. Stop by and visit us at Laurel Hall, located at the south end of the Green!
