Enrollment/Degree Verification Request Form


Current students can download and print their enrollment verification by following the steps below. This self-service, secure document provides the proof needed to verify your enrollment (including for insurance and loan companies).

Login to your UDSIS Student Homepage. Click either the Courses and Enrollment tile or the Grades & Transcripts tile to find a link to Enrollment Verification.

For students who have graduated, the official transcript serves as the official document for verifying your degree and GPA. For more information, please visit our Official Transcript site. Transcripts can be ordered 24 hours a day, 7 days a week, and your request will normally be processed within the next business day. Ordering an official electronic transcript is the university’s quickest option for verifying your GPA or degree.

If an official transcript and/or UDSIS enrollment are not sufficient, you may need an official letter to verify your enrollment. In these cases, the Registrar’s Office can provide a letter printed on University letterhead with Registrar signature: see Enrollment Verification Sample   Degree Verification Sample. To request an official letter, complete the form below.

Processing time for verification letters is typically ten business days after our receipt of this form.
