You have been identified as being enrolled in one of the Highmark Delaware plans (IPA/HMO or CDH Gold), which will no longer be available as of the new plan year effective 7/1/2017 as outlined in the May 10 email.

If you do not make an election, you and/or your dependent(s) will automatically be defaulted into the equivalent Aetna plan.

Please make an election by logging on to http://www.udel.edu/flexnet by Friday, May 26th.

Please complete a spousal coordination of benefits policy form, if you are covering a spouse: http://ben.omb.delaware.gov/documents/cob/

Please complete an Aetna dependent child coordination form, if enrollment is changing to Aetna: http://ben.omb.delaware.gov/documents/cob/aetna-dependent-cob.pdf
