ACHA DIVISION III SEASON START-UP CHECKLIST

- Elect someone from your team that will serve as your "Hockey Information Director".  This person needs to have access to the email address listed on your ACHA application form.

- Your Hockey Information Director will be directly responsible for sending in the following information at the required dates.  Failure to submit any of the below listed information could effect your team’s eligibility for the National Tournament:

- Send proof of USA Hockey insurance, or equivalent coverage by October 31, 1999.  You will not be included in the rankings until this information is received.

- Accurately complete and fill in your team’s eligibility form for the Fall semester.  The deadline for the fall eligibility form is October 31, 1999.  Please remember that:
1.  All information, except the player signature MUST BE TYPED!
2.  The registrar must both sign the form and affix a school stamp/seal.
3.  Faxed copies are not acceptable.  Original documentation must be sent.
4.  For the second semester, each team is required to resubmit this form.  Many teams in the past have not.  Your team will not be eligible for the National tournament unless that form is submitted.  Not in contention?  If you do not submit this form for the second semester, your program will not be eligible for play the FOLLOWING season.
5.  You will not be included in the rankings until this form is received.
- Each player must complete and sign an "Annual Agreement to Participate" form.  The ACHA has provided 25 copies.  Make additional copies as necessary.  These forms are due back by October 31, 1999.  You will not be included in the rankings until these are received.

- As soon as it is complete, send your 1999-00 player roster and schedule.  This information is due on October 1, 1999.  This information is due early to help update the web page as soon as possible.  Please include the following information:
1.  Players’ number, position, height, weight, hometown and school year.
2.  Current list of all coaches. Managers, trainers, advisors and other team personnel.

- Remember to report all weekend scores by Sunday evenings at 11:00pm Eastern Time.  Scores should be sent via email to:  Dblevins99@aol.com     If you do not have access to your email over the weekend, please call the scores in to (302) 831-1350.  When you submit your scores you must report any players from your team that received game DQ penalties.  These will be posted the following Monday along with the weekend scores.

- RANKINGS:  All scores from your region will be forwarded to your region coordinator.  You will be receiving region ranking information from your region coordinator as the season progresses.

- ACHA Statistics will be kept this season.  You will be required to fax in all scoresheets.  Fax all score sheets to Dan Blevins at (302) 831-4037.  Scoresheets are due each Tuesday afternoon.
